What Is an Immigration Physical Exam?
The immigration medical evaluation includes multiple areas that must be completed by your exam provider and verified by a civil surgeon.
- Testing for communicable diseases – Individuals will undergo lab testing for tuberculosis, syphilis and gonorrhea. Testing for syphilis and gonorrhea are only done for individuals over the age of 15.
- Checking for physical or mental disorders – Following DSM and WHo guidelines, your provider will ask questions to determine if any physical or mental disorders are present. The presence of a disorder does not disqualify a person from the immigration process. Individuals may apply for waivers if any disorder is present.
- Assessment for drug addiction or abuse – Via medical history and questions asked by the provider, it will be determined whether you have a drug addition or participate in drug abuse. Abuse or addiction alone may not be grounds for disqualification as waivers are available for some instances.

Which Vaccines Do I Need for My I-693 Form?
Per the USCIS, I-693 forms must show individuals have received the following vaccines:
- COVID-19
- Hepatitis A
- Hepatitis B
- Hib
- Influenza
- Measles, mumps and rubella
- Meningitis
- Pertussis
- Polio
- Pneumonia
- Rotovirus
- Tetanus and diphtheria
- Varicella
These immunizations are to be given following ACIP guidelines. This means some vaccines may not be required based on age or current medical condition. Be sure to check with your immigration physical exam provider on what you may need.
Do I Need an Immigration Medical Exam for My Green Card?
Yes, an immigration medical exam is required as part of the I-693 form. This medical form must be signed by the civil surgeon who performed the exam within 60 days of filing Form I-485. The I-693 form must show proof of vaccination for all required immunizations. The USCIS must then make a decision within two years of Form I-693’s submission.
What Is the I-693 Form?
The I-693 form, Report of Medical Examination and Vaccination Record is a required document as part of the USCIS immigration process. The form requires a physical, proof of vaccination, meeting with a civil surgeon and some forms of testing. The form should be submitted with Form I-485 with a signature from the civil surgeon no more than 60 days old at the time of submission.

How Do I Complete the I-693 Form?
Individuals receiving an immigration physical for Form I-693 are to complete Part 1 of the form only. All other parts of the I-693 form will be filled out during the immigration physical exam and through the completion of lab work.

Will I See a Civil Surgeon for My Immigration Physical?
Yes, immigration physicals are conducted under the direction of civil surgeon’s per USCIS requirements. Immunizations and testing may be done by nurses or other qualified staff, but document review and approval will be conducted by a civil surgeon.
